About Us

Our mission is to effectively meet the needs of individuals with attention deficits and learning problems by offering thorough and accurate evaluations, as well as a variety of non-medical intervention services.  Though we appreciate the role medication can play in the treatment of ADD/ADHD, we value the use of education, clinical services and support before trying medication, or in conjunction with medication.

Insurance Companies

Julia Frishtick participate with the following insurance companies:

Dr. Butnik does not accept insurance or work directly with insurance companies or other third-party payers. This makes Dr. Butnik an “Out of Network" provider. 

If you would like to know if your insurance policy includes Out of Network  reimbursement benefits you can:

  • Call the Member Services number on the back of your insurance card to determine if your policy allows you to work with Out of Network providers. If so you can ask: 
    • How much is the deductible and how much has been met this year?
    • What is the Out of Network deductible for outpatient behavioral health?
    • Ask for an estimate of what their reimbursement rate is for the following services:
      • Initial evaluation - CPT Code 90791
      • Ongoing therapy - CPT Codes 90834 and 90837
      • Psychological testing - CPT Codes 96130 and 96131
      • Test interpretation - CPT Code 90887
    • Ask for a copy of the form you will need to complete to submit your request for reimbursement with your Superbill.

If you use Out of Network behavioral health benefits...

  • Payment in full is due at each appointment.
  • We will provide a Superbill (an itemized receipt with a diagnosis code) for you to file with your insurance.
  • You fax or mail the Superbill to your insurance company with their filing form.
  • Depending on your Out of Network benefits, your insurance company may apply the cost of services toward your deductible or may provide direct reimbursement to you.

Fees and Payment Policy

The No Surprises Act (NSA) protects consumers from unexpected medical bills. It includes a Good Faith Estimate (GFE) provision, which requires providers to give cost estimates to uninsured or self-pay patients. If a bill exceeds the GFE by $400 or more, patients can dispute it with the U.S. Department of Health and Human Services (HHS). 

Fee schedule with CPT Codes included for each procedure (CPT codes are used for insurance purposes): 

  • Initial session (90791): $200.00 (typically one hour)
  • Individual therapy (90834): $160 (38 to 52 minutes); (90837): $185 (53 to 60 minutes)
  • Family Therapy: (80846, 90847): $160 (38 to 52 minutes), $185 (53 to 60 minutes)
  • School consultation: $200.00 per hour (we charge for preparation time, meeting time, and travel time to and from the school)
  • Psychological testing: (96130, 90131): $200.00 per hour (charges include face-to-face time as well as report preparation – typically one hour). Total test time varies based on the nature of the evaluation
  • Test Interpretation: (90887): $200.00 per hour (usually one hour)
  • Legal consultation: $250.00 per hour (charges include preparation time, travel time, and time spent testifying)
  • Telephone calls: no charge for brief calls, but the usual hourly therapy rate may be charged for lengthy and/or ongoing telephone communications.
  • Correspondence: no charge for brief notes, but the usual hourly therapy rate may be charged for lengthy or ongoing written communications, or for completion of lengthy forms 

Payment is expected at the time of service. For clients with insurance, charges include deductibles and co-payments. For minor children, we expect fees to be paid by the parent who initiates the appointment.  

For services provided by Ms. Frishtick we will submit charges to your insurer, and we will wait up to 90 days for your insurer to respond. After that time, any charges not paid by your insurer will become your responsibility. 

Services provided by Dr. Butnik, once paid, can be sent to you as an itemized superbill that you can submit to your insurer.
